La revue en ligne \u00ab Globalization and Health \u00bb a publi\u00e9 un article intitul\u00e9 \u00ab Health in the Sustainable Development Goals : Ready for a Paradigm shift ? \u00bb (La sant\u00e9 dans les objectifs de d\u00e9veloppement durable : pr\u00eate pour un changement de paradigme ?) Les objectifs actuels comprennent la r\u00e9duction de la mortalit\u00e9 infantile et n\u00e9onatale et d’autres marqueurs de sant\u00e9 importants. Les nouveaux objectifs de d\u00e9veloppement durable propos\u00e9s par les Nations unies se concentreraient davantage sur le d\u00e9veloppement du leadership pour la collaboration intersectorielle, la pr\u00e9vention et le renforcement de l’engagement civique (voir le r\u00e9sum\u00e9 ci-dessous et l’article en ligne ici<\/a>). <\/p>\n

Pour nous, m\u00e9decins, c’est un d\u00e9fi permanent (et un changement de paradigme) que d’adopter le type de leadership qui nous fera passer d’un mod\u00e8le strictement m\u00e9dical et scientifique \u00e0 un mod\u00e8le plus holistique, impliquant les aspects relationnels, comportementaux, \u00e9motionnels et spirituels de la sant\u00e9 et des soins de sant\u00e9. C’est un d\u00e9fi ambitieux mais important. <\/p>\n

Les missions m\u00e9dicales et sanitaires chr\u00e9tiennes ont fourni une bonne partie des services de sant\u00e9 dans les r\u00e9gions pauvres en ressources. S’ils ne doivent pas concurrencer les services gouvernementaux, ces efforts missionnaires peuvent les compl\u00e9ter et \u00e9tablir des partenariats avec eux – et c’est souvent ce qu’ils font. Elles peuvent et doivent fournir des occasions d’explorer et de collaborer pour le type de leadership dont nous avons besoin pour nous attaquer aux \u00e9normes probl\u00e8mes abord\u00e9s par ces nouveaux objectifs de d\u00e9veloppement durable. [Je parle ici des efforts missionnaires chr\u00e9tiens qui se d\u00e9veloppent sur le long terme avec une sensibilit\u00e9 culturelle et un leadership local]. Par exemple, des approches nouvelles et cr\u00e9atives en mati\u00e8re de pr\u00e9vention, de soins de sant\u00e9 et d’encadrement sont issues de ces efforts ; j’ai vu un certain nombre de cas o\u00f9 les gouvernements ont \u00e9t\u00e9 en mesure d’amplifier ces efforts plus modestes. C’est un d\u00e9fi qui m\u00e9rite d’\u00eatre relev\u00e9. <\/p>\n

Les objectifs du Mill\u00e9naire pour le d\u00e9veloppement (OMD) ont galvanis\u00e9 l’attention, les ressources et la responsabilit\u00e9 sur un petit nombre de probl\u00e8mes de sant\u00e9 dans les pays \u00e0 revenu faible et interm\u00e9diaire, avec des r\u00e9sultats sans pr\u00e9c\u00e9dent. La communaut\u00e9 internationale \u00e9labore actuellement une s\u00e9rie d’objectifs de d\u00e9veloppement durable qui succ\u00e9deront aux OMD. Cette \u00e9tude examine la base factuelle des propositions actuelles relatives \u00e0 la sant\u00e9 en ce qui concerne la charge de morbidit\u00e9 et la faisabilit\u00e9 technique et politique des interventions visant \u00e0 atteindre les objectifs. Contrairement aux OMD, le programme de sant\u00e9 propos\u00e9 aspire \u00e0 \u00eatre universellement applicable \u00e0 tous les pays et est suffisamment large pour englober \u00e0 la fois les maladies transmissibles et non transmissibles, ainsi que les charges \u00e9mergentes dues, entre autres, aux accidents de la route et \u00e0 la pollution. <\/p>\n

Nous soutenons que la r\u00e9alisation de l’agenda n\u00e9cessite un changement de paradigme dans la mani\u00e8re dont nous abordons la sant\u00e9 mondiale afin de relever cinq d\u00e9fis : 1) assurer un leadership pour la coh\u00e9rence et la coordination intersectorielles sur les moteurs structurels (y compris sociaux, \u00e9conomiques, politiques et juridiques) de la sant\u00e9 ; 2) passer du traitement \u00e0 la pr\u00e9vention par le biais d’approches locales et politiquement intelligentes \u00e0 un programme beaucoup plus large ; 3) identifier des moyens efficaces pour s’attaquer aux d\u00e9terminants commerciaux de la mauvaise sant\u00e9 ; 4) int\u00e9grer davantage les approches fond\u00e9es sur les droits ; et 5) renforcer l’engagement civique et assurer la responsabilisation.
