Notre doyen, le Dr Carl Taylor, dirigeait un d\u00e9partement passionn\u00e9 par la r\u00e9duction des maladies et la promotion de la sant\u00e9 gr\u00e2ce aux soins primaires. Mais les donn\u00e9es relatives \u00e0 cette approche \u00e9taient limit\u00e9es. Comme beaucoup d&#8217;autres, j&#8217;ai constat\u00e9 que le r\u00eave \u00e9tait charmant, mais qu&#8217;il n&#8217;\u00e9tait pas si facile \u00e0 mettre en \u0153uvre lorsque je me suis rendu en \u00c9thiopie.   <\/p>\n<p>J&#8217;ai donc \u00e9t\u00e9 encourag\u00e9 d&#8217;apprendre qu&#8217;il existe aujourd&#8217;hui plus de 700 \u00e9tudes dans la litt\u00e9rature m\u00e9dicale qui soutiennent les soins de sant\u00e9 primaires \u00e0 base communautaire. Le Dr Henry Perry et d&#8217;autres chercheurs de Johns Hopkins ont publi\u00e9 leurs conclusions dans le Journal of Global Health <a href=\"http:\/\/jogh.org\/community-based-PHC.htm\">.<\/a> <\/p>\n<p>Il est prouv\u00e9 que le CBPHC est une composante importante d&#8217;un programme de sant\u00e9 maternelle et infantile con\u00e7u de mani\u00e8re globale, non seulement pour ses effets directs sur la morbidit\u00e9 et la mortalit\u00e9 maternelles et infantiles, mais aussi parce qu&#8217;il contribue \u00e0 une utilisation appropri\u00e9e des installations hospitali\u00e8res et cliniques. En d&#8217;autres termes, nous avons besoin d&#8217;un \u00e9quilibre entre les soins curatifs, pr\u00e9ventifs et promotionnels. Les faits sont clairs.  <\/p>\n<p>Voici quelques aspects du CBPHC :<\/p>\n<ul>\n<li>Engagement des groupes de femmes<\/li>\n<li>L&#8217;innovation comme les spectacles de marionnettes<\/li>\n<li>Travailleurs et b\u00e9n\u00e9voles locaux r\u00e9mun\u00e9r\u00e9s et non r\u00e9mun\u00e9r\u00e9s<\/li>\n<li>Renforcement du personnel des centres de sant\u00e9 p\u00e9riph\u00e9riques<\/li>\n<li>Faire appel \u00e0 des professionnels de la sant\u00e9 et \u00e0 des non-professionnels de mani\u00e8re compl\u00e9mentaire<\/li>\n<li>Visites r\u00e9guli\u00e8res \u00e0 domicile<\/li>\n<li>Gestion de cas au niveau communautaire<\/li>\n<li>\u00c9quipes mobiles<\/li>\n<\/ul>\n<p>Le Dr Perry souligne que la croyance en la m\u00e9decine curative est tr\u00e8s puissante ; nous sommes parfois hypnotis\u00e9s par la technologie et les soins curatifs. Bien qu&#8217;ils occupent une place tr\u00e8s importante, nous savons \u00e9galement qu&#8217;un grand pourcentage de maladies peut \u00eatre att\u00e9nu\u00e9, pr\u00e9venu ou trait\u00e9 par des moyens non m\u00e9dicaux. Nous voulons, bien s\u00fbr, offrir des soins cliniques de qualit\u00e9, mais nous devons \u00e9galement d\u00e9ployer des efforts soutenus et intentionnels pour avoir un impact sur les communaut\u00e9s par des moyens plus larges et moins \u00ab techniques \u00bb. Le CBPHC n&#8217;est peut-\u00eatre pas technologiquement sophistiqu\u00e9, mais il peut \u00eatre puissant.   <\/p>\n<p>En termes de justice sociale, Perry souligne que le CBPHC peut avoir un effet \u00ab pro-\u00e9quit\u00e9 \u00bb, en \u00e9vitant les barri\u00e8res potentielles aux soins de sant\u00e9 que les institutions elles-m\u00eames peuvent avoir.<\/p>\n<p>Alors que nous faisons des disciples dans nos missions de soins de sant\u00e9, souvenons-nous d&#8217;\u00e9lever \u00e0 la fois les professionnels de la sant\u00e9 et les non-professionnels, le personnel qui travaille dans les institutions et celui qui travaille dans les communaut\u00e9s. Les besoins \u00e9tant pressants autour de nous, nous devons assurer le bon fonctionnement des h\u00f4pitaux sans pour autant y consacrer toutes nos ressources, mais en \u00e9quilibrant les soins cliniques (tertiaires et secondaires) et les soins de sant\u00e9 primaires au sein de la communaut\u00e9. Nous avons besoin non seulement de m\u00e9decins, mais aussi de travailleurs sociaux, de promoteurs de la sant\u00e9 et de grands-m\u00e8res qui sont pr\u00eats \u00e0 faire le sacrifice d&#8217;aimer et d&#8217;investir dans les communaut\u00e9s afin d&#8217;apporter la gu\u00e9rison \u00e0 ceux qui ont le c\u0153ur bris\u00e9 et l&#8217;espoir \u00e0 ceux qui souffrent autour de nous.  <\/p>\n<p>Si nous sommes \u00e9trangers \u00e0 une culture (expatri\u00e9s ou missionnaires), nous ne devons pas \u00eatre na\u00effs quant \u00e0 la difficult\u00e9 d&#8217;une telle approche. Elle touche souvent \u00e0 des valeurs, des r\u00eaves et des approches culturelles profond\u00e9ment ancr\u00e9s, qui sont d\u00e9form\u00e9s par notre propre r\u00e9bellion \u00e0 l&#8217;\u00e9gard de Dieu et des voies de son royaume.
